This session explores how case studies can be used to document the process and outcomes of community health initiatives to showcase the work being done in the community and serve as a model for others taking on similar work. Presenters will focus on three different case studies recently completed for the Minnesota’s Statewide Health Improvement Program grant in rural Cottonwood, Jackson, Faribault, Martin and Watonwan Counties. The agency embarked on the case study process to document program successes and lessons learned, and to inform other agencies doing similar work across the nation. Case studies were completed for initiatives that aimed to create healthier worksites and increasing physical activity in schools. In this session, presenters will share why case studies were used and their experiences designing instruments, collecting data, and creating case study reports.
