Presentation Abstract: This project took place in an organization which serves approximately 3.5 million people. The goal of the project was to develop an ongoing process to monitor and evaluate services. The current development focused on care related to patients with heart failure. Using the literature, process and outcomes indicators were developed. The stakeholder group agreed to use a performance dashboard format to monitor and evaluate their services. Challenges arose during the implementation stage. Main challenges include incongruent data sources, multiple data source gatekeepers, time lag in receiving the information, and organizational needs and resources. The presentation will focus on these challenges and how the project team addressed the issues. Ideas on how to mitigate and manage these issues would be discussed as part of the presentation.
