Attached are materials for Eval12 Poster "Evaluation As Intervention" (When Evaluation Is The Intervention) Abstract: This poster will describe the California HealthCare Foundation’s (CHCF) experience developing and using a common reporting template to track the progress and impact of the Regional Information for Shared Excellence (RISE) Initiative. Working through four intermediaries, the initiative includes eleven regions and over one hundred community clinics across California that are working together to improve clinical quality, operational efficiency, and patient experience. Because a major goal of the initiative is to build capacity for data-driven improvement, CHCF incorporated data collection, reporting, and analysis into the intervention. Thus, CHCF’s Research & Evaluation department partnered with program staff to develop a data reporting template that could be customized for each of the eleven regions, as well as clear processes for data collection and reporting.
