The road from evidence-based health intervention to improved health and wellness for individuals and communities needs clear guideposts, lights at strategic points and critical “You Are Here” signs — not only at the end, but along the way. This coffee break session will talk about community readiness assessment and the development of evaluation approaches that are co-authored by community members as well as researchers and practitioners. Taking a life cycle view of intervention, we’ll look at an action readiness model, a program logic model and a tool for community-designed expectations development as the foundation for evaluation.
