This presentation will describe a case study of two community health initiatives that used evaluation findings over a four-year period to shape and improve their programs. For each initiative, a brief description of how the evaluation recommendations from two distinct evaluation studies were applied to the next iteration of the program will be provided. Key facilitators, barriers and lessons learned common to both initiatives will be shared to provide a "how-to" for other evaluators to optimize the application of their evaluation results and help to improve programs. #EvaluationUse #2014Conference #HealthEvaluation
